I knowingly and willingly consent to receive nail services performed by Gloss Nails during the COVID-19 pandemic.

To help prevent the spread of infection and to help protect each other, I understand and will follow public health and safety guidelines and will adhere to Gloss Nails COVID-19 policies.

I understand that due to the visits of other clients, the characteristics of COVID-19, and the process of nail services, that I have an elevated risk of contracting COVID-19 by merely being in the Gloss Nails studio.

I understand the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms and still be highly contagious. It is impossible to determine who has it and who does not, given the current limits in virus testing.

I verify that I have not traveled outside the United States In the past 14 days.

I will not come to my appointment with any of the following symptoms of COVID-19: fever, chills or sweats, body aches, shortness of breath, unexplained rash, loss of sense of taste or smell, dry or congested cough, runny nose, or sore throat. If I believe I have any symptoms, I will cancel or reschedule my appointment.

I have read, fully understand, and completed this questionnaire truthfully. I agree that this constitutes full disclosure and that it supersedes any previous verbal or written disclosures. I understand that this document is to ensure the safety of our community and will be used for contact tracing purposes should the need arise.

RELEASE OF LIABILITY: The risks of COVID-19 are not well understood and there is controversy among experts on how the virus can spread and there is difficulty in scientifically determining whether anyone has the virus at any moment in time. In consideration for providing services, by signing below I agree to accept all responsibility for the risk that I may contract COVID-19. By implementing new safety and sanitation procedures, Gloss Nails cannot guarantee that any of these measures will completely protect me from contracting COVID-19. I agree that if I take any steps to make a claim for damages against Gloss Nails, I shall be obligated to pay all attorney's fees and costs incurred as a result of such claim. I acknowledge that I can go elsewhere to receive nail services. By signing this waiver and release, I acknowledge that I am free to go to other salons who may not require my agreement to accept responsibility for contracting COVID-19. GLOSS NAILS RESERVES THE RIGHT TO REFUSE SERVICES TO ANY CLIENT THAT IS SICK IN ANY WAY OR VISIBLY PRESENTS SYMPTOMS AS DESCRIBED ABOVE.
