I am over the age of 18 and acknowledge that I choose to receive services from Gloss Nails and have voluntarily filled out this consent form.

I acknowledge that I am receiving services from a licensed professional and will conduct myself appropriately while in the nail studio, including limiting the use of any devices (cell phone, laptop, etc.)

To the best of my knowledge, I have no health condition or allergies that would prevent me from receiving nail services.

I grant permission for Gloss Nails to use and/or apply any necessary products as part of the service I am receiving. I understand that nail products contain reactive chemicals which may result in allergic reactions in some individuals and hereby assume all risk of personal injury which I may sustain as a result of the application of any of the products used, or the manner of service rendered, including the application of chemicals and use of appliances or implements including clippers, nippers, e-files, or other tools and materials.

I acknowledge that no guarantee has been made about the results of my services and/or treatments. I agree that the service I receive is satisfactory and final after payment is rendered. I must notify Gloss Nails within 24 hours of service if a problem arises or if a fix is needed. For any changes or fixes/repairs thereafter, I understand that I will be charged accordingly.

I agree that Gloss Nails reserves the right to cancel or reschedule my appointment if I present with any nail or health conditions that may be contraindicated for receiving nail services. I understand that it is out of the scope of the nail technician to diagnose health conditions or to give any medical advice.

I authorize Gloss Nails to take photos before, during, and after my nail service and authorize the use of these photos solely for the purpose of Gloss Nails internet and portfolio publication, social media, or promotions and advertising. I acknowledge that the Gloss Nails studio is monitored by a 24-hour video security system and that I may be recorded at any time while on the premises.

By signing below, I am aware of and acknowledge any unexpected results or issues that may arise with the nail services I receive. I am fully aware of any risks involved in the services I receive, including any adverse reactions or allergies. I agree to voluntarily release, forever discharge, and agree to indemnify and hold harmless Gloss Nails from any and all losses, claims, injuries, or damages, sustained or incurred by me arising out of or related to the services and/or treatments rendered by Gloss Nails or results thereof, including but not limited to any claims arising from the treatment of my nails, the application of any nail products to my nails, including, but not limited to, claims based on any allergic reaction, damage or injury to my nails or skin, or any other claim and shall be responsible for all attorney's fees and costs incurred as a result of such claim.
